The School of Social Sciences, Humanities, and Arts is committed to providing advising that is supportive, safe, and confidential. While advising sessions are typically one to one, students may bring an additional person into the advising session if they wish to do so. By signing this form, you are acknowledging the following:


    ·        The student wavies their right to privacy of otherwise confidential information
        protected under the Family Education Rights and Privacy Act (FERPA), including,
        but not limited to

o   GPA

o   Course schedule

o   Degree completion status

o   Registration status

o   Class standing

o   Major and Minor

o   Course history

o   Academic standing


·       The advisor reserves the right to ask a guest to wait in the lobby until this form is signed by the student.  


The student is responsible for the conduct of their guest. Should the conduct of the guest impose an undue burden that prevents the delivery of academic advising services, the advisor may conclude the meeting at any time or ask the guest to wait in the lobby.
